Submitting Tax Exemption Form (Ohio)

If your business qualifies for sales tax exemption, it’s very easy to submit a form and have taxes removed from current or future orders. Please note we cannot refund taxes on orders that have already been processed. So be sure to submit this form before placing your order!

Note: We do not consult you on whether your business qualifies to be tax-exempt. Please speak with a licensed tax professional to see if your business qualifies.

  1. Fill out the latest Ohio Sales and Use Tax Unit Exemption Certificate

  2. For the Vendor’s Name, write Fraga Graphic Solutions, LLC

  3. In the larger text box under “… the activity of the purchase, or both, as shown hereon:” write in the reason for claiming exception or exemption.

    1. If you are unsure, consult with your tax professional regarding your tax exemption reason.

  4. In Purchaser’s name, write your business name

  5. In Purchaser’s type of business, write your business type

    1. If you are unsure, consult with your tax professional regarding your business type.

  6. Fill in your address in the Stree address, and City, state, ZIP code boxes

  7. Sign the form

  8. Date the form

  9. Provide your Vendor’s license number, if any

  10. Save the form and email it to sales@fragagraphics.com or your customer service representative.

Please allow up to 24-72 hours for the tax exemption status to be applied. Our team will notify you once it has been set up.
